This site is your portal for requesting laboratory safety inductions, training and maintenance within the School of Clinical Sciences at Monash Health. 

Use the Submit a Request tab at the top right for the following forms

Induction and training requests:

• OHS Inductions for laboratories and specialised facilities (incl. radiation laboratories and liquid nitrogen storage facilites)
• Gas cylinder and Cryogenics practical training
• Autoclave training

Use the MMC building access request form for proxy card access at MMC.

Equipment or facility maintenance requests:

• Scheduling repairs or preventative maintenance of equipment
• Notification of a facilities defect
